Zero3 was merely the first game to DISPLAY your average.  Your average was always tracked by the game, and was always the basis for Rank.  It's the same method across all four games.

bad design? well, Spiral Pegacion had a nice design artwork wise, but his sprite was pretty awkward. Especially his attack one. (when he throws the tornado at you. that one was his worst sprite) and the fact that he was worse animated than even Storm Eagle.

It's because the Rank reflects your average across all missions, not the most recent.

You want to talk bullshit ranks, check out Mario Kart Wii.  B Grade for a freaking perfect score.

Huh. That's really odd. I managed to keep a consistent A rank in Z1 despite having some sloppy runs on a few stages there as well. (Most notably the bomb defusing mission; got sick of fighting Phantom because at the time I didn't realize you were actually able to hit him during his desperation move like Leviathan and Harpuia, yet still got an A rank anyway.)

Also, lol @ Mario Kart Wii; all the more reason why you should stick to F-Zero X for your racing fix.

Meanwhile, how about the 8 mavericks from X3?  All of them Neon Tiger and maybe Crush Crawfish were either ugly or stupid looking.  ESPECIALLY Big-Mouth Catfish.

Volt Man would've been a pretty good design if it would've got a lot of polish IMO.
