Michael J. Fox was accidentally hanged, passing out for 30 seconds until anyone on the set noticed. Eli Wallach was nearly decapitated in The Good, The Bad And The Ugly, as he really did lay underneath a moving train in one scene. If he raised his head at an incorrect time, well, you know.
Leatherface from The Texas Chainsaw Massacre had to wield a real chainsaw and swing it around, and he
can't see anything through his mask. He slipped at one point, and the running chainsaw landed dangerously close to his face.
And of course, there's Jackie Chan, who's just batshit; preforming ALL his stunts and breaking almost every bone in his body TWICE. Including a scene in Police Story where he slid down a pole covered in lights, causing 3rd degree burns to his face and hands, nearly paralyzing him. Adding the lights to the pole was Jackie's idea, even though everyone on the set stressed how dangerous it was.
